Services are provided by our trained and certified nurse assistants who can provide and assist with personal care, assist ambulation or exercise, and activities of daily living.
Medical Social Services
Our Licensed Medical Social Worker will assess our client's emotional and social needs and assist them & their families in utilizing community resources such as transportation, meals on wheels, and for those who qualify for the homemaker program.

WHAT IS HOME HEALTH CARE?
Home health care offers a wide range of nursing therapy, medical, and support services that can be provided in the comfort of your home. This type of home care is appropriate for those suffering from chronic illness, recovering from an acute injury or illness that may need skilled care to stay at home. Home health care can also help to prevent or shorten hospital stays and is cost-effective.
We believe that patient & family participation helps in early recovery and progress towards our patient's independence.
Who will qualify for home health care services?
Clients who require intermittent medical care by skilled care professionals.
A Physician's evaluation or encounter and order for home health care services are needed.
Must be essentially homebound.
Who will pay?
Medicare and commercial insurance will cover the home health care provider.
